Proforma and Definitions
Data Collection
Data for the SMSR are collected in the main by MS nurses across Scotland. Data are collected on paper proformas and submitted to the Scottish Healthcare Audit team either electronically or by courier. Data are entered onto our central database where they are stored according to NSS security and privacy policies.
Proforma
The SMSR dataset was reviewed and implemented in April 2017.
